Welcome to Partner For Care (“Partner For Care,” “we,” “our,” or “us”).
These Terms and Conditions (“Terms”) govern your access to and use of the Partner For Care
website, platform, and related services (collectively, the “Platform”).
By accessing or using Partner For Care, you agree to be bound by these Terms.
If you do not agree, please do not use the Platform.
1. Purpose of Partner For Care
Partner For Care is an educational and informational resource designed to support caregivers
of individuals receiving hospice care. The Platform provides guidance, tools, and educational
materials intended to help caregivers feel informed and supported throughout the hospice journey.
Partner For Care does not provide medical care, diagnosis, or treatment and is not
a substitute for professional medical advice, hospice services, or clinical decision-making.
2. Eligibility and Access
Access to Partner For Care is provided through participating hospice organizations.
Hospices enroll in a subscription plan and may grant caregivers access to the Platform.
If you are a caregiver, access is typically provided by invitation from your hospice.
If you do not yet have access, please contact your hospice provider to request an invitation.
Hospice organizations are responsible for managing caregiver access associated with their account.
3. Educational Use Only – No Medical Advice
All information provided on Partner For Care is for educational and informational purposes only.
- Content may include guidance on basic care practices, medications, and symptom management.
- Content may be created or reviewed by medical professionals but does not replace advice from your hospice team.
- Always consult your hospice care team, physician, or licensed healthcare provider regarding medical decisions.
Partner For Care is not responsible for medical outcomes resulting from the use or interpretation
of information provided on the Platform.
4. User Responsibilities
By using the Platform, you agree to:
- Use Partner For Care only for lawful purposes
- Provide accurate information when registering or accessing the Platform
- Maintain the confidentiality of your login credentials
- Not misuse, copy, scrape, or redistribute Platform content without permission
You may not use the Platform in any manner that could damage, disable, or impair its functionality.
5. Accounts and Invitations
Invitation links are intended only for the caregiver to whom they are issued and should not be shared.
Partner For Care reserves the right to revoke or suspend access if misuse, unauthorized sharing,
or violations of these Terms are detected.
6. Intellectual Property
All content on Partner For Care, including text, graphics, logos, educational materials,
and platform design, is the property of Partner For Care or its content providers and is protected
by applicable intellectual property laws.
You may not reproduce, distribute, or create derivative works from Platform content without
prior written consent.
7. Availability and Modifications
We strive to keep Partner For Care available and accurate; however, we do not guarantee uninterrupted
or error-free access.
Content, features, and services may be updated, modified, or removed at any time without notice.
8. Limitation of Liability
To the fullest extent permitted by law, Partner For Care shall not be liable for any direct,
indirect, incidental, or consequential damages arising from your use of the Platform.
Use of the Platform is at your own risk.
9. Privacy
Your use of Partner For Care is also governed by our Privacy Policy, which explains how we collect,
use, and protect your information.
10. Termination
We reserve the right to suspend or terminate access to the Platform at any time if these Terms
are violated or if access is no longer authorized by a participating hospice.
11. Governing Law
These Terms are governed by the laws of the State of California, without regard to conflict
of law principles.
