TRANSFERING/MOVING PATIENT

Transferring a patient in bed requires care and attention to ensure their safety and comfort.
Here are step-by-step instructions to guide you through the process

Preparation

  • Wash your hands thoroughly and put on gloves if needed.
  • Explain the procedure, ensuring they understand and feel comfortable with the process.
  • Adjust the bed to a safe working height. Lower the bed if necessary to minimize the risk ofinjury during the transfer.

Positioning

  • Ensure they are lying comfortably in the center of the bed with their head supported by apillow.
  • If the patient is able to assist, instruct them to bend their knees and place their feet flat on thebed to provide stability during the transfer.

Assessment

  • Assess any medical devices or tubes and ensure they are secured and not pulled out duringtransfer.
  • Assess if they are in pain and need medication prior to moving.

Transfer to side (from lying flat on back)

  • Stand on the side of the bed opposite to the direction you want to move them.
  • Lower the bed rail (if present) on the side where you're standing to create an open space.
  • Use the "draw sheet" under them to gently move them towards you so there is enough roomfor them to lie on their side.
  • Place one hand under the patient's shoulders and the other under their knees, keeping theirbody aligned and supporting their weight evenly.
  • Gently roll the patient away from you, moving them onto their side facing away from you.
  • Reposition yourself on the other side of the bed, maintaining support under the patient'sshoulders and knees.

Repositioning

  • If necessary, adjust their position to ensure they are comfortable and well-supported withpillows placed strategically to prevent pressure ulcers and maintain proper alignment.
