Your hospice will provide you with a link to access the site. Just follow the instructions in your welcome email or brochure to get started.

If you are registering as an individual and not through a hospice, please follow the instructions on the site to complete your registration.

If you are accessing Partner for Care through your hospice, your account will be set up automatically when your loved one is admitted. You'll receive a login link via email, and your password will simply be your cell phone number.

If you are registering as an individual and not through a specific hospice, you will be prompted to create your own account by following the instructions on the site.

No. Partner for Care is entirely web-based. You can use it on any smartphone, tablet, or computer with internet access.

Using the Platform

Partner for Care is designed primarily around clear, easy-to-understand infographics to help you quickly grasp complicated caregiving and medical topics. Alongside these visual guides, many pages include short videos that supplement the written content, making key subjects easier to understand. For deeper, more comprehensive information, you can ask Care Ai, our chat agent trained to provide expert-backed answers.

The platform provides general guidance for many common caregiving situations. For anything specific to your loved one's condition, please contact your hospice nurse directly.

Chat Agent Support

Care Ai is the Partner for Care platform's chat agent, trained specifically to answer any medical or caregiving questions related to patients in hospice. It relies exclusively on trusted, curated information developed and reviewed by the Partner for Care clinical team.

Simply type your question into the chat interface, and Care Ai will provide expert-backed answers based on hospice-approved content. Unlike other chatbots, Care Ai does NOT search the internet or pull from unverified sources, ensuring accurate, relevant responses.

Yes. Care Ai does not store or share any personal, medical, or identifying information. All answers come solely from the internal knowledge base curated by Partner for Care.

You can ask Care Ai about symptoms, medications, caregiving techniques, hospice services, and what may be going on with your loved one's condition. For personalized, urgent, or complex medical questions, please contact your hospice nurse directly.

General Questions

Yes. The site allows access for up to three caregivers. The primary caregiver can provide the hospice with names and contact information of two additional users. Those users will then receive their own login details.
